Prerequisites

Letter of Authorization (LOA) Signed by the authorized user and/or number owner. The information on the LOA must match the authorized user name and service address held by your current carrier.

A scanned or electronic copy of the most recent phone bill/Invoice (not older than 30 days) that clearly shows: Account number, Authorized user’s name and All numbers to be ported
